For all appointments, please call (786) 268-6200 or click here to request an appointment online. We typically only see 1 injured body part per appointment in order to provide the best patient care in a timely manner.
Please arrive 10-15 minutes early for your appointment for registration. You will be asked to fill out a health questionnaire. We can email you the forms in advance or you may fill them out upon arrival. Appointments will be expedited if you are able to send us your completed forms in advance.
When you come for your appointments, please remember to bring the following:
- Driver's license or State ID
- Insurance information
- Copies of operation records, medical records, recent x-rays, recent MRI images and reports, CT scans, etc., from prior doctor visits
- Medications list
- If you have had surgery elsewhere, please bring a copy of your operation report
Often during the course of your evaluation or treatment for an orthopedic condition, we will require x-rays to determine the cause of a problem or to evaluate your progress.
To cancel an appointment:
Telephone the office during business hours Monday-Friday between 8:30am-4:00pm. Please cancel at least 24 hours in advance so that your appointment time can be allocated to another patient.
Due to the large volume of patients we see and in order to respect the time of other patients, if you are more than 30 minutes late for your appointment you will be asked to reschedule. We also ask that you call the office prior to your appointment time to let us know if you will be late. In efforts to decrease your wait time, we hope all our patients understand the importance of arriving on time/early.
